Step 1Fill out this 2011 Key Code Application / Renewal Form online. Each requestor must complete one.
Step 2Returning and first-time customers need to attach or mail readable copies of the following THREE items:
1. Acceptable photo ID ( a valid driver’s license, an ID card issued by the DMV or equivalent, or a passport)
2. Company license (a valid business, occupational, professional, or sales tax license with the company name)
3. Proof of employment (employee badge, business card, employee verification letter, paystub, or W-2 form with both your name and the company name)

WE WILL REJECT ALL FAXED ITEMS because most are unreadable.
Step 3Wait 3 to 5 business days for your identity and employment to be verified.
